After you complete this form, the person you are nominating will receive an email letting them know you nominated them and inviting them to accept the nomination and request a meal.

It is important that you do not fill out a request on their behalf, only submit the nomination. We are not able to process requests that have been submitted on behalf of someone else.

If you do not want the person to know you nominated them, you can put “a friend” in the name field.

If you do not have an email address for the person you want to nominate, you will be unable to submit a nomination form.
